Abstract

Fatores humanos são um dos principais fatores contribuintes para acidentes na aviação comercial ao longo de sua história. Buscando tornar o voo mais seguro, diversos sistemas automatizados foram sendo desenvolvidos. Pilotos automáticos com número crescente de recursos, autothrottle, navegação por GPS, sistemas de alerta em caso de aproximação com o solo (GPWS), entre outros. A automação inegavelmente contribuiu para tornar a aviação mais segura, porém, mesmo aeronaves altamente automatizadas sofrem acidentes. Entender as razões por trás de um acidente é o caminho para evitar novos acidentes. O presente artigo discute o acidente do voo TAM 402, ocorrido em 1996 na cidade de São Paulo. Diversos fatores contribuintes estiveram presentes, tais como: falha no sistema de controle do reverso, falha em diferentes sistemas de proteção ligados ao reverso, falhas de treinamento da tripulação, dificuldade dos pilotos em lidar com falhas do autothrottle, e incapacidade para compreender a origem da instabilidade da aeronave e assim garantir o voo seguro. O foco da tripulação no sistema de autothrottle, deixando de compreender que a falha era de abertura do reverso, fez com que seus esforços para recuperar a aeronave se mostrassem inúteis. Compreender o comportamento da tripulação do voo TAM 402 é pertinente ainda hoje, quando dificuldades na relação tripulação x sistemas automatizados continuam presentes em inúmeros acidentes.
